About the service
The service can visit you in your home to provide a bespoke and outcome based service, helping customers lead fulfilling lives and remain as independent as possible. The service offers from as little as one hour a week support up to 24 hour a day care packages.
This service might be right for you if you:
- Have learning disabilities
- Have a physical disability
- Have a sensory impairment
- Have a memory impairment
- Have dementia
- Have mental health needs
- Have had an acute medical illness or accident
The service can provide a range of different support such as:
- Increasing independence
- Personal care
- Getting dressed
- Taking you medication
- Shopping
- Meal preparation
- Education and employment options
- Social activities
Every customer has a care package that is tailored to your own unique requirement and linked to needs identified by an Adult Social Care Assessment where appropriate.
The service is regulated by the Care Quality Commission to ensure that it maintains high standards of care.
Referrals
Referrals for the service follow an initial assessment of needs from the North Essex Adult Social Services. They decide what kind of help you may need and how long for, and refer you on to the service most suitable to your needs.
