About the service

The service offers support to customers that live independently in the borough of Hackney with a mental health diagnosis. Please see our referral form for additional criteria.

Predominantly we offer emotional support as well as advice around tenancy matters, welfare benefits, attending activities to reduce social isolation, education, training and employment and we can refer onto other specialist agencies that may be of assistance to you.

Staff visit you in your own home or in the community on a fortnightly basis to discuss your support needs and assist you in achieving the goals you have set in your support plan. We work with you to help you gain independence and improve your health and wellbeing.

Our working hours are 9.00am – 5.30pm Monday to Friday. Please call us on 0300 123 1339 during these hours for further information.

Referrals

We accept referrals via GPs, Mental Health Practitioners, Hospitals or any other support agency as well as self-referrals.
