Hornsey Lane is a service offering medium to long term accommodation and support with personal care to older adults with a variety of mental health needs.
The service offers support to a maximum of 12 customers, both male and female.
We support you with your daily living skills including nutrition, personal care and medication. The overarching aim of the service is to provide a safe and secure environment in which you are given the opportunity to reach your full potential and, where appropriate, the opportunity to move on to a more independent living environment. We aim to achieve this by supporting you to construct daily routines to help maintain or improve your mental wellbeing.
You make choices and decisions for yourself, and where you might need additional support with this, we make sure that your best interests are taken into account when decisions about your finances or welfare are made on your behalf by appointed individuals.
We aim to support you to be active in your community and pursue your interests. In addition, we support you to overcome any discrimination and prejudice you might face.
Staff at Hornsey Lane work with you, carers and relevant agencies to identify your needs and develop a regularly reviewed holistic care plan.
The service also works in conjunction with external professionals to offer sessions aiming to increase physical activity (for example, community based physical activity such as walking and yoga). There are also opportunities for physical activity through shopping, housework and gardening.
