About the service

The service offers housing, along with specialist support for people between the ages of 18 and 65 who have mental health issues or learning disabilities and are in need of help to develop skills to live independently.

Wakefield is located in Greenhithe with amenities, shops, surgeries and local buses close by.

You meet regularly with your support worker and the support provided is tailored to your individual needs but may include maintaining your tenancy, advice on paying your bills, setting up direct debits, applying for other benefits and linking in with healthcare and local community groups. We also provide information on social activities and organise group activities for customers.

Your support worker provides you with support to access specialist services relating to your needs. Links are also made with the local community mental health team and you are sign posted to any other appropriate agencies.

The service lasts for around two years and, once you are able to live independently, we support you to apply for housing through the council’s Homechoice service and bid for available properties in the area. After customers move on from the service, we are able to offer re-settlement support for up to three months.

Staff can be contacted by phone 9.00am – 5.00pm, Monday to Friday and they visit the scheme to offer direct support to customers once a week.

Referrals come through Dartford Borough Council.
